Immigration Medical Exam in Chicago and Suburbs

An Immigration Medical Exam, including vaccinations, is required when people want permanent and lawful resident status in the United States. It is necessary to submit I-693 form completed by a designated Civil Surgeon for people who are filling for citizenship, immigrant visas and some non-immigrant visas, refugees and adjustments of status to that of a permanent resident. Immigration Medical Exams also called “Green Card Medical Exam” or “USCIS Medical Exam”.

Civil Surgeon

USCIS (United States Customs and Immigration Service) conducts medical examinations for those applicants who seek statutory and Green Card in the USA. For this they designate Civil Surgeon or a USCIS doctor to conduct medical tests with full authorization to give necessary vaccination to foreigners applying for adjustment of status or Permanent Resident Status (Green Card). Civil Requirements of I-693 Form

I-693 is a form required for immigration process. There are some requirements following to complete the Immigration Physical Exam and fill out form I-693:

Medical Examination by Certified Civil Surgeon:

The medical examination is necessary for all the people who apply for immigration. This examination includes all the physical tests and checkups of general health like blood pressure, temperature, pulse rate, heart, lungs, eyes and ears. The civil surgeon will review medical records related to previous vaccinations received, previous tuberculosis tests, and any other medical records.

Completion of USCIS required Vaccinations:

Vaccinations are the crucial part of immigration process, you should have all the data of required vaccinations which ensured by the civil surgeon. Some vaccines are important and required for Immigration and Nationality Act, and others are required because the Centers for Disease Control and Prevention (CDC) have determined they are in the interest of public health. In order to enter into the country as a green card holder you must receive the following vaccinations:

1. Mumps, measles, rubella
2. Polio
3. Tetanus and diphtheria toxoids
4. Pertussis
5. Haemophilus influenza type B
6. Hepatitis B
7. Varicella
8. Influenza
9. Pneumococcal pneumonia
10. Rota-virus
11. Hepatitis A
12. Meningococcal

If you’ve already been vaccinated, then bring your vaccination reports to the doctor designated by USCIS. Reports must be in English or translated by an official translator per USCIS requirements. If you haven’t been vaccinated then the civil surgeon will manage them for you.

Completion of Required Blood & Urine Testing:

This is another and important phase of medical testing for immigrant visa. This testing is required especially for detecting two diseases, Syphilis and Gonorrhea, which are the sexually transmitted, contagious and very common diseases in the world. Sometimes applicants may not show any sign or symptoms of the disease so the test is required to ensure that anyone who is infected must be treated and will not be contagious in the USA.                                          

Syphilis Testing

Syphilis testing is done through a blood test called an RPR.
• If the results are normal, then there is no Syphilis and exam is completed.
• If the results are not normal, then it means the laboratory will do a second test with same blood sample to confirm about the reality of results. This second is known as T. Pallidum-PA. If this test shows as abnormal too, then the applicant should be treated for Syphilis with proper medication before finishing the examination forms.

Gonorrhea Testing

Tests for Gonorrhea are done through a urine test.
• If the results are normal, then that portion of the examination is complete.
• If the results are positive or not normal, then the applicant should be treated with the proper medication.

Completion of Tuberculosis Testing:
USCIS conducts another medical test which detects Tuberculosis. This is also an important test to make immigration safe in the USA. Tuberculosis (TB) is disease which is caused by bacteria called Myobacterium tuberculosis. This disease has a direct effect on lungs and gradually effects other the parts of body like the kidney, the spine and the brain. If not treated properly, TB can be fatal.
TB is declared as a serious disease because it once was the cause of many deaths in  the United States. This disease is easily transmitted from person to person via coughs, sneezes and speaking. Through these means, bacteria transferred into human bodies, causing some individuals to show low symptoms and some to show high (active) symptoms. Individuals who carry mild symptoms of TB can take medication to prevent further development of the disease.
USCIS requires the test is done under the supervision of a civil surgeon. Since 2003, the number of TB cases have been decreasing, but Multidrug-resistant TB (MDR TB) remains a concern and the extensively drug-resistant TB (XDR TB) has become an important issue. Previously, TB was diagnosed by Tuberculin Skin Test (TST), but now civil surgeons use IGRA test by using a blood sample. The IGRAs that are currently acceptable to the CDC are: the QuantiFERON ® TB Gold Test, the QuantiFERON® TB Gold in Tube Test, and the T-Spot ® TB Test. The results of the IGRA test are generally available within 24 hours of your office visit. If the result is negative, you will probably not need any further testing for TB. If the result is positive, you will be required to have a chest x-ray as a means of additional screening for TB.

Completion of I-693 paperwork:
I-693 form is completed after full medical examination which fill out in the presence of a civil surgeon. The applicant can only fill such a section which is allowed by the USCIS. Instructions for completing the form can be found here.

The civil surgeon will finalize all reports including a report and examination of the chest X-ray. The civil surgeon will place all the I-693 related paperwork in a sealed envelope. DO NOT OPEN THE SEALED ENVELOPE. You are required to submit the envelope with your immigration application.
